Welcome to the REF Lookup Tool. 

Use the tabs below to search by RHC service (HCPCS/CPT) codes or revenue codes. Then use the type, description, and code filters to tailor your search. Here are some helpful hints:

• Use the type filter to search the data by type of clinic – “Independent/Freestanding RHCs” or “Provider-Based RHCs.” The default search is for all RHCs.
• Use the code filter to search for specific codes using the alphanumeric characters. You can search for a range of codes by using the first few alpha numeric parts of the code.
• Use the description filter to search for codes by their descriptions.
• Print or download and save your filtered results of the full dataset as an Excel or CSV file.

LIMITATION

As described on the previous page, this website is intended as a helpful aid for RHC stakeholders to use as a quick lookup and reference guide on the HCPCS/CPT codes used by RHCs to describe the services provided to beneficiaries covered by the traditional Medicare program during each claim year. The fact that RHCs in the sample used these codes to describe services provided to traditional Medicare beneficiaries does not imply that RHCs received reimbursement for each service as many were provided under the all-inclusive rate (AIR) payment methodology used by RHCs. The data in the lookup tool provides an estimate of HCPCS/CPT codes use by all RHCs (based on the codes reported by RHCs for services provided to traditional Medicare beneficiaries represented in the 5% sample.) We cannot vouch for the accuracy of all the codes used. We recommend that RHCs conduct appropriate due diligence with Medicare, Medicaid, and commercial payers prior to using any of the codes reported in this file.
